Recruit your walking team        \n\n2. Complete walk forms           \n\n3. $25 to walk\n\nForms available at:  YMCA\, Chamber of Commerce\, Maquoketa State Bank & Clover Ridge Place  You can also request one of these places to mail you a form as well.
